What is Semaglutide?
Semaglutide is a glucagon-like peptide receptor agonist (GLP-1 RA), that is included in a class of antidiabetic agents called incretin mimetics, which are natural signals that our body uses to control glucose. Originating from the gut, GLP-1 RA is a bioidentical peptide that our body’s naturally use to regulate glucose and enhance insulin secretion.
How does it work?
In addition to enhancing glucose metabolism, Semaglutide also slows gastric emptying, which makes you feel full. It can cause significant weight loss and therefore a reduction in the risk of cancer, diabetes and heart disease in those who are obese or overweight. It is indicated as a tool to use in addition to therapeutic lifestyle changes that include healthy dietary and exercise habits.
How long do I use it for?
Semaglutide is a treatment option that can be used long-term (ie > 12 mo.). According to the American Association of Clinical Endocrinologists and American College of Endocrinology (AACE/ACE) Obesity Clinical Practice Guidelines, weight loss medications should be offered as chronic treatment along with lifestyle modifications to patients with obesity when the potential benefits outweigh the risks. Short-term pharmacotherapy has not been shown to produce longer-term health benefits and cannot be generally recommended.
Who is a good candidate?
The following criteria may indicate a potential good candidate for Semaglutide therapy:
 

  • A BMI of 30 or greater
  • Overweight (defined as a BMI >27) with a weight-related condition, such as high blood pressure, diabetes type 2, or abnormal cholesterol levels.
  • Type 2 diabetes or who have laboratory signs of prediabetes, insulin resistance or metabolic syndrome.
  • Abdominal waist circumference greater than 35” (for women) or 40” (for men).
  • A waist to height ratio greater than 0.5
  • Patients who currently practice healthy lifestyle habits, or are willing to implement these habits, such as:
Who is not a candidate?

The following criteria are indicative of those whom may not be good candidates for Semaglutide therapy: 

  • A personal history of Type 1 diabetes, diabetic retinopathy or currently using other diabetic medications.
  • A personal or family history of medullary thyroid carcinoma (MTC)
  • A personal history of Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)
  • A personal history of pancreatitis or gallbladder disease
  • If you are pregnant or plan to become pregnant.
  • If you are breastfeeding or plan to breastfeed.
  • If you are currently sedentary and/or do not consume a healthy diet with no desire to implement healthy lifestyle changes
Potential Side Effects

The most common adverse reactions are typically gastrointestinal in nature and can cause nausea, abdominal pain, diarrhea or constipation, decreased appetite, vomiting and constipation. Serious side effects may include thyroid tumors, including cancer. Please tell your healthcare provider if you experience any hoarseness, difficulty swallowing, swelling in your neck or shortness of breath.
